    This form is designated for special funds. General and Title I and II funds stipends should NOT be submitted using this form.
    Entering a stipend funded out of the General Fund prevents this form from submitting.

    If you need to submit a stipend and aren't sure what form to use, please refer to the Stipend Handbook or contact Payroll.
