**New Leave Forms

  • All staff members are expected to use the new Request for Leave and Absence Report Forms. These new forms replace all leave forms that may be in use at your school or within your department.  The Request for Leave form is to be used BEFORE you take the leave. The Absence Report Form should be used AFTER an absence.  

    • Please fill the form out electronically (if possible).
    • Print the form, sign the form and submit to your supervisor for approval.
    • If approved, supervisor should return a copy to you and alert department/school time keeper.
    • If denied, supervisor should advise you of the reason(s).
