OKCPS has partnered with Community Health Centers of Oklahoma to serve our students who may be at risk for substance abuse and/or depression.  Please read this letter in its entirety to find out more.  If you do NOT wish for your student to be screened in this program, please PRINT and SIGN this letter.  Letters for students who wish to opt out need to be returned to Putnam Heights Academy no later than Wednesday, September 22.  If you have any questions or concerns, please contact us by phone or email.
