OKCPS Families, 

The Strengths and Difficulties Questionnaire (SDQ) is now available for students, teachers and families through October 6, 2023. If you have students in grades 1-12, you should have received an email from Rhithm App with a link to the survey. If you have not received an email from the Rhithm App, please check your Junk inbox or contact your child's school. 

Who will take the survey?

• The SDQ survey will have two respondents for elementary students (parent and teacher)

• The SDQ survey will have three informants for middle and high school students (parent, student, and teacher)

  • Middle and High School students will access the survey through the students’ Clever account

• Families can respond to the SDQ through the Rhithm/Securly link that families should receive via email

Why take the survey?

The district will utilize the data collected:

• To create plans and prioritize services at the school site level for students with elevated risk

• To provide parents with relevant resources tailored to their student’s need

• To gauge the overall mental health “temperature” of each school site and the district at large

• Parents will be provided a “parent report” that will provide them with information about their student and contact information for assistance and support.
