OKCPS Families,
The Strengths and Difficulties Questionnaire (SDQ) is now available for students, teachers and families through October 6, 2023. If you have students in grades 1-12, you should have received an email from Rhithm App with a link to the survey. If you have not received an email from the Rhithm App, please check your Junk inbox or contact your child's school.
Who will take the survey?
- The SDQ survey will have two respondents for elementary students (parent and teacher)

The SDQ survey will have three informants for middle and high school students (parent, student, and teacher)

Middle and High School students will access the survey through the students’ Clever account

Families can respond to the SDQ through the Rhithm/Securly link that families should receive via email
Why take the survey?
The district will utilize the data collected:

To create plans and prioritize services at the school site level for students with elevated risk

To provide parents with relevant resources tailored to their student’s need

To gauge the overall mental health “temperature” of each school site and the district at large

Parents will be provided a “parent report” that will provide them with information about their student and contact information for assistance and support.
