Pre-K and Kindergarten Round-Up
Underway for
Oklahoma City Public Schools
OKLAHOMA CITY
– If you have four or five-year-olds it’s time to start
planning for the fall as OKCPS Pre-K and Kindergarten
enrollment is underway from Monday, May 1 through Friday, May
12. Children who are five-years-old on or before September 1,
2006 are eligible for kindergarten. Children who are
four-years-old before September 1, 2006 may enroll in OKCPS’
numerous four-year-old programs. Tuition is free for the
four-year-old program. Children who are three-years-old on or
before September 1, 2006, may enroll in the tuition-based
three-year-old program where space is available. The cost for
the tuition program is $6.00 per day.
To
enroll parents must bring their child’s Birth Certificate;
Immunization records; Proof of address - utility bills i.e. gas,
electric, water (telephone bills are not accepted); Home
telephone number plus emergency and work telephone numbers.
Pre-Kindergarten Immunization
Requirements:
Immunization records must clearly
indicate which immunizations the child has been given, dates
administered, and signatures or stamp of physician or agency
giving immunization. The following immunizations are required:

Four (4) DTaP (Unless the fourth dose was given
after the 4th birthday)
·
Three (3) OPV (Unless the third dose was given
after the 4th birthday)
·
One (1) MMR with first dose given after 12
months of age
(Measles,
Rubella, and Mumps)
(Not
administered after the fifth birthday).
·
Three Hep B
·
Two Hep A+
·
One Varicella (or proof of chicken Pox)
Kindergarten Immunization Requirements:
Immunization records must clearly
indicate which immunizations the child has been given, dates
administered, and signatures or stamp of physician or agency
giving immunization.

Five (5) DTaP or Dt (Unless the fourth dose was
given after the 4th birthday)
·
Four (4) Polio (Unless the third dose was given
after the 4th birthday)
·
Two (2) Measles, Mumps and Rubella (MMR) given
on or after the student’s first birthday. A second MMR
vaccination is required before the student enters kindergarten.
A student who has just started the immunization series is on an
acceptable schedule as outlined by the State Department of
Health.

3 Hepatitis B
·
2 Hepatitis A
·
1 Varicella (Chicken Pox)
