1. ALL of the following forms make up the complete admissions packet. These forms are available in two formats:
Microsoft Word for Windows® format, version 6.0. (This is an older version which should be compatible with most modern word processing programs in use since 1995 including WordPad)

- Student Application
- Authorization for the release of academic records
- Statement of residency
