• Student Eligibility Grade Check Sheet  This form is to be completed by Wednesday 6th period of every week by the student and teachers. Students will pick up form from their 6th period teacher on Monday and return it on Tuesday to the same teacher, a student aide will pick up forms on Wednesday from the teacher and give it to the Instructional Facilitators for completion of the Ineligibility list for Thursday's roster.
