BEHAVIORAL CONTRACT FOR

Classen School of Advanced Studies DANCE STUDENTS

 

This contract is between ____________________________________, a Classen dance student, the Classen Dance Department and the Classen SAS administration.

It is effective from this date of August 21, 2008 through May 27, 2009, or until this student’s withdrawal from all Classen dance classes.

 

 

I, ________________________________ as a student member of the Classen SAS Dance Department, have read and agree:

          1. To follow the Attendance Policy set forth by Classen SAS Dance

              Department and supported by the Classen Administration.

          2. To follow the Dress Code set forth by Classen SAS Dance Department and

              supported by the Classen Administration.

          3. To follow the instructions of my teachers, the first time given and to accept

              their position of authority as the director of the class, rehearsal or

              production.

          4. To communicate all needs or questions in an appropriate, non-aggressive,

              respectful manner with all my instructors.

          5. To respect and support my fellow dance peers at all times.

          6. To respect the school facilities, costumes and equipment.

          7. To commit to participating in all required performances and rehearsals.

          8. To be evaluated and graded by all Classen Dance teachers and understand

              the Dance Department’s grading policy.

          9. I understand that if I do not comply with Classen Dance Department

              policies and/or the Classen SAS assertive discipline plan that I can expect

              the following Teacher Consequences For Disruptive Student Behavior, as

              stated below and in the Classen SAS student handbook:

                   Step 1. Two verbal warnings from the teacher.

                   Step 2. Third verbal warning, student will not be allowed to participate

                               in the remaining dance activities for that class. The student

                               will receive an F for his/her daily grade and will have a private

                               conference with the teacher.

                   Step 3. Teacher contacts parent by phone (or mail if phone number is

                               unavailable).

                   Step 4. Conference with teacher, student, counselor and parent.

                   Step 5. After school detention or school service and parent conference.

Step 6. Referral to assistant principal (documentation must be listed

                              on the evidentiary report form that must accompany all

                              referrals).
