Medicines At School

This is to advise and clarify the Columbus Enterprise Elementary School's medication policy.  No medicines (prescription and/or over the counter) of any kind will be given to a student at school, unless a written authorization signed by the parent/guardian is on file in the school clinic or accompanies the medication sent to school per Oklahoma Law and Oklahoma City Board of Education policy.

***Compliance with the above and following instructions is required:

  1. All medicines (prescription and/or over the counter) must be brought and checked in with the school nurse or school office.

  2. Only medicines for life threatening conditions will be allowed to be carried by a student during the school day.  NO EXCEPTIONS!  In order to carry a medicine at school, it must be accompanied by a copy of the release for self-administration of medication signed by a parent/guardian and a physician stating it is necessary for medicine to remain with the student.  The original signed form must be on file in the school clinic.

  3. All medications must be in the original prescription container or over-the-counter container labeled with: the date, name of prescriber, name of student, name of student, name of medicine, and directions for administration.

  4. Send only enough medicine to be given at school, keep the rest at home.  The container will be returned when empty.
