JOM Services

 To enroll your child in our JOM Indian Education program:

  1. Your child must have Certificate Degree of Indian Blood (CDIB) documentation OR tribal enrollment verification to qualify for our JOM services. 

  2. Complete the appropriate form of the service you are requesting

  3. All assistance programs will require a receipt of costs before purchase and an itemized receipt after purchase. (Please read each form carefully for instructions)

Title VII Services

To enroll your child in our Title VII Indian Education program:

  1. Parent/Guardian must submit a completed “Title VII 506 Form”
  2. Student must qualify for free or reduce lunch program
